RELEASE and WAIVER of LIABILITY

Assumption of Risk: I assume risk of injury or harm from my use of the GCC or while performing tasks as a GCC volunteer including any risk that may arise from negligence or carelessness on the part of GCC from dangerous or defective equipment or property owned, maintained, or controlled by them. I agree to comply and observe all posted rules and further to obey all verbal instructions and warnings given by any representative of GCC. Further, I understand it is not the responsibility of GCC or its agents to teach safety rules, nor is it the function of GCC to serve as guardian of my safety. Further, I understand that I am responsible for repair of any damage to the facility or its furnishings that I cause while using the facility.
Waiver and Release: I waive and release forever, discharge, and hold harmless GCC and its successors and assigns from any and all liability, claims, and demands of whatever nature, either in law or in equity, which arise now or hereafter from my use of the GCC. I understand and acknowledge that this release discharges GCC from any liability or claim that I may have against GCC with respect to bodily injury, personal injury, illness, death, property damage, or actions of any kind that may occur to me or my child while using the facility.
Insurance: I understand that GCC does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. I expressly waive any such claim for compensation or liability on the part of GCC.
Medical Treatment: I hereby release and forever discharge GCC from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency while at GCC.
Media Release: I give GCC permission to take and use photographs, images, video, or audio recordings of me or my children while at GCC. We value your privacy and you may opt-out by specifically informing GCC.
Children: GCC welcomes children to use its facilities and services and contribute as volunteers. I understand that GCC staff and volunteers are not responsible for the supervision, safety or behavior management of children at GCC. I understand that children will be asked to leave if they are unable to comply and observe all posted rules and further to obey all verbal instructions and warnings given by any representative of GCC.
In this Release and Waiver of Liability form, Gustavus Community Center (GCC) includes the GCC facility, directors, officers, employees, volunteers, representatives, and agents, and an activity or class instructors.
By signing below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
